NFPA 20 would be the document that I would reference for this information. I did not find a min/max dimension in a quick review of the standard.

I think I looked many years ago and never found anything. I find it helpful to always ask an inspector when I'm presented with a choice on where or how to install a test header. They have a very practical approach and will think about it differently than an installer or designer.

NFPA 20 does not contain a specific code reference that sets a minimum elevation requirement for the fire pump test header above grade. The height at which the test header should be positioned above grade is influenced by local building codes and AHJ requirements.

The test header just needs to be accessible. I've seen them in all kinds of odd places but always able to be accessed by an average person standing on the ground i.e no ladder required.
