Assuming that it's been approved by a structural engineer, is there any code or standard restrictions that would prevent running CPVC branch pipe through a built-up floor beam?

Certainly, as long as the structural engineer approves of the approach.

common sense usually dictates cutting holes in beams is a last resort. can't imagine a situation where this is the last resort.

Recently had a job where 14' ceilings weren't enough. We coordinated penetrations in brand new steel beams!!

Also, if this is a seismic job, don't forget about additional clearance that may be required, which could make for an even larger hole through the beam. Most beams we've penetrated go by the third rule. Penetration has to be no more than 1/3 of the depth of the beam, located in the middle 1/3 of the beam vertically, and located within the middle 1/3 of the span. As you stated though, only with very clear instructions, in writing from the structural engineer (preferably the EOR and not one that you or the GC hired). If this is a retrofit situation and there is not a structural EOR, the structural engineer should be hired as far up the chain as possible. If the GC is smart, they will have the owner hire the structural engineer, or at least the architect.

Running CPVC through anything: I’d get a letter of compatibility for any adhesive or FR treatment in the beams. It doesn’t have to be in intimate contact to cause trouble.

It is allowed but must be way-off the centre of the built up floor beam.
