We are working on a hospital project where the underground service pipe to the pump room was buried at 4 feet (1.2 m). Are we required to provide a concrete sleeve or any special concrete provisions to protect the HDPE pipe?

A10.4.3.1.2 NFPA 13,24 (2016) Sufficient Clearance should be provided when piping passes beneath foundations or footers.

10.4.3.1.2 Piping shall be installed a minimum of 12 in. below the bottom of the building foundations or footers

It is a good practice to protect the pipes going underneath a building to avoid that ground motion (earthquake, settlement, downward movement after heavy rains...) may damage / break the pipe.
