For an entry lobby area that is open to the floor above with a elevator on the second floor, and there is a pitched skylight for the roof. Where do you locate the smoke detector for elevator recall? I could not find any specific requirements for smoke detector location requirements in NFPA 72.

This is an excellent question, and I come across it often. Please refer to NFPA 72, 2010 or 2013 edition, 17.7.3.1.3 and 17.7.3.1.4. This will lead you to the Annex which suggests, for ceilings greater than 15', you may mount the detector within 60" of the top of each elevator door opening to protect the opening. This is allowed because it is not being used for general detection, but rather, for the protection of the opening.

How many floors does the elevator serve? More than just the main and "mezzanine" you indicated?

The elevator serves the basement, 1st and 2nd floors. The first and second floor entry area is the areas that are open to one another, and where the skylight is located.
