Is a fire supply allowed to have any joints, offsets, or fittings underneath the building foundation other than the 90-degree elbow looking up to the flange?

Dan's response is spot on. There is a provision that trenches can be used if you need to place additional pipe lengths below a building.

I think the OP is asking about fittings other than the 90 looking up. I have seen... specs that require the only fitting allowed to be the one 90, but honestly, as wise as that may be, I cannot find that written in any standard - just the 10 ft. limitation.

To further clarify, the footings are typically at the building edge below exterior walls. Joints are not permitted directly under bottom of the footing. Design prudence generally minimizes joints under slabs though they allowed to 10 ft...without an open, accessible trench. I like NFPA 24 (2022) Figure A.10.4.3.1. If UG joints on the lead-in must be within the building footprint, put them under the slab, with allowable sleeving through the wall above the footing, or in an accessible trench. As has been stated by others, there are other provisions for how to accomplish this with varying degrees of potential facility disruption which need to be considered. Don't forget necessary considerations for the lead-in to be properly- and fully restrained.
